These Terms govern your use of lifechange.health and the services offered through it by LifeChange Longevity ("LifeChange," "we," "us"). By using the site or our services, you agree to these Terms.
Not for emergencies
Our services are not for medical emergencies. If you think you have a medical emergency, call 911 or go to the nearest emergency room.
Eligibility
You must be at least 18 years old and located in a state where our services are available. You agree to give complete and truthful information.
Medical services
LifeChange Longevity is not a medical provider. Medical services are provided by independent, licensed healthcare providers, who use their own judgment to decide whether a treatment is appropriate for you. Completing an assessment does not guarantee a prescription. Information on this site is for general education and is not medical advice.
Telehealth consent
By requesting an online visit, you consent to receive care through telehealth, including secure messaging, phone and video. Telehealth has benefits and limits: a provider cannot physically examine you, and technology problems can cause delays. You may be asked for lab work, identification or a video visit, as required by your provider or your state. You may decline telehealth care at any time.
Compounded medications
Some treatments are compounded medications prepared by licensed pharmacies for individual patients. Compounded medications are not reviewed or approved by the U.S. Food and Drug Administration (FDA) for safety, effectiveness or quality. Results vary from person to person and are not guaranteed.
Prices, subscriptions and billing
Prices are shown at checkout. Treatment plans renew automatically at the frequency you choose until you cancel. You can cancel future renewals at any time before your next billing date by contacting us. Promotional offers apply to new patients only, are limited to the first month unless stated, and may change or end at any time.
Refunds and shipping
- Not approved: if a provider decides a treatment is not right for you, you receive a full refund for that treatment.
- After a prescription is filled: medications cannot be returned or refunded once they have been prepared or shipped, because of pharmacy safety rules, except where the order arrives damaged or incorrect.
- Shipping: medications ship in discreet packaging, temperature-controlled where required. Delivery times are estimates. Contact us within 7 days of delivery if an order arrives damaged, incorrect or missing.
Communications
You agree that we and our partners may contact you by email, phone and text about your account, orders and treatment. Marketing messages are governed by our Privacy Policy, and you can opt out at any time by replying STOP.
Acceptable use
You agree not to misuse the site, provide false information, obtain medication for someone else, or resell any medication.
Intellectual property
The site's content, brand names and design belong to LifeChange Longevity or its licensors and may not be copied without permission.
Disclaimers and limits of liability
The site is provided "as is." To the fullest extent the law allows, LifeChange Longevity is not liable for indirect, incidental or consequential damages, and our total liability for any claim is limited to the amount you paid us in the 12 months before the claim. Nothing in these Terms limits liability that cannot be limited by law.
Governing law
These Terms are governed by the laws of the State of Wyoming, without regard to conflict-of-law rules.
Changes
We may update these Terms. The effective date above shows when they last changed. Continuing to use the site means you accept the updated Terms.
